A collaborative partnership between NHS Imperial College, AstraZeneca and the agency I worked with came together with an aim to create a more integrated pathway for heart failure care and transform the clinician and patient experience - an urgency exacerbated by Covid-19.
My role as a Lead Consultant involved mapping both clinician and patient journeys, reviewing technology providers and managing the launch and implementation of the digital solution. Working closely with clinicians and the technology provider, we launched a digital remote monitoring app for patients and successfully transformed many manual operations and processes.
The project has since been shortlisted across two categories for HSJ Awards in 2022 (results to be announced in November 2022).
